Scheduling an Appointment with Us

Once you are enrolled in the CDCP, simply call our clinic to schedule an appointment. On the day of the appointment, make sure to bring your Sun Life insurance letter and a photo ID.

Service Canada Instructions for Eligible Canadians

Over the coming months, according to the schedule outlined above, individuals who may be eligible have received and will continue to receive an invitation letter from Service Canada outlining the steps to enroll in the CDCP.
Steps to follow:

  • Call Service Canada at 1 833 537-4342 and press 1. It is also possible to visit a Service Canada center to make this request.

  • Have on hand two (2) pieces of identification and the code indicated in the top right corner of the letter sent by Service Canada. This code is unique and personalized, and is reserved for the recipient until April 30, 2024. To make a request after this date, you will need to visit the website Canada.ca/dentaire.

  • Have on hand the social insurance number of the person to be enrolled.

  • The individual will need to answer questions about their dental coverage to ensure they are not covered in any way by a private plan.

Once the person's eligibility has been validated, they will receive an information package from Sun Life Canada containing a membership card and the start date of their dental care coverage under the CDCP.
